Between moving to new areas and brand new enrollments, day services are often not selected until after residential placement is established. In a Host Home, an individual with an intellectual or developmental disability chooses to live in a qualified provider's home. Provides oversight of the services and helps connect the individual and their provider with community resources.
WHAT IS A HOST HOME PROVIDER?
Effective March 15, 2022, an IntelliRide Transportation Certificate of Compliance must be attached to new and maintenance applications. Electronic Visit Verification is required with this enrollment and will be automatically added with an approved application for Health First Colorado (Colorado's Medicaid Program). Recommendation of approval from the Colorado Department of Public Health and Environment to provide Specialty 676 Respite CES/SLS and a Home Care Agency Class A or Class B license – if applicable.

Our host home providers are an exceptional team of caring, dedicated, and compassionate people that provide safe, inclusive, and fulfilling home-life opportunities for individuals with intellectual/developmental disabilities. Laradon’s Adult Host Home program provides individualized living situations to people who can benefit from living in the private home of a family or individual. Individuals enjoy living in a family setting, engaging in community activities and sharing household responsibilities. Host Home Providers create a caring support system and assist the individual with personal care, meals, finances, medical appointments and community connections. Level of care determinations are made annually by the case management agencies .
This automated sensing provides caregivers the ability to improve safety, enhance the quality of life, and increase options for greater independence for all individuals requiring support.
