Submitted charges cannot be more than charges made to non-Health First Colorado-covered individuals for the same service. The base procedure is the procedure with the highest allowable amount. The base code is used to determine the allowable amounts for additional CPT surgical procedures when more than one procedure from the same grouping is performed. When multiple services are performed, the primary reference letter for each service should be listed first, other applicable services should follow.

The coordinator ensures that the individual’s needs are being met in the home and that their plan of care is being followed. They will visit the individual and the Host Home Provider in the home regularly. Cheyenne Village Nurses will coordinate healthcare for the individual and the Benefits Coordinator will manage Medicare, Medicaid, and Social Security benefits.

The Host Home Provider is charged with providing a safe, loving environment for you. They are responsible for day to day care including providing meals, medication administration, arranging for medical care and treatment, and to meet other specific needs you might have. In the family care-giver model, family members can become Dungarvin employees and are paid to provide supports for their family member.
How does RoundUp Fellowship support its Host Home Providers?
Prior authorization only assures that the approved service is determined to be necessary and is considered a benefit of the Health First Colorado. A resume that demonstrates at least one year of experience in the provision of music therapy to pediatric/adolescent clients for each individual providing services. Electronic Visit Verification is required for some services billed under this specialty code and will be automatically added with an approved application for Health First Colorado (Colorado's Medicaid program). For both SSN and EIN enrollments a massage license through the Colorado Department of Regulatory Agencies is required for each individual performing services.
Rural travel add-ons may be billed for members residing in counties designated as rural or frontier. This work includes monitoring the effective and efficient provision of services across multiple funding sources. Colorado Department of Public Health and Environment Home Care Agency Class A or Class B license is required only for providers interested in providing members training on Personal Care. If services are provided in a county/city that does not require a Contractor's license, please attach a signed attestation on letterhead that states the county/city served does not require a Contractor's license.
